Application forms will not be processed without all of these documents. Please ensure that you have scanned, digital versions to upload with the application form.

• A recent colour, head and shoulders, passport/ID-size photograph of your child

• Copies of the following:

  • your child’s unabridged birth certificate

  • your child’s immunisation/clinic card

  • ‘Road to Health’ book

  • your child’s latest pre-primary or primary school report

  • both parents’/guardians/sponsors identity documents

  • proof of permanent residence

  • valid residence and study permits, if not a South African citizen.

  • divorce agreement, legal guardianship or death certificate, if applicable

  • most recent SARS returns for both parents/guardians
